Determining Your Health Class for Tobacco and Nicotine Users

NOTE: Use the following tables if you HAVE USED cigarettes, pipes, chewing tobacco, snuff, nicotine patches or nicotine gum in the last 12 months.

These tables offer general guidelines as to how most companies will classify you (standard tobacco, preferred cigarette, or standard cigarette) based on medical and personal issues. The number of classes and class criteria varies by life insurance company. If you do not meet one or more of the requirements listed in the standard tobacco or preferred cigarette columns, please select "standard" when calculating your quotes. If you have a health impairment, higher table rates than standard may apply. Underwriting requirements vary by company.
